Designated Coordinator – Tyler
Job Summary: Supervision of Direct Support Staff, development of outcomes and methodologies for each individual served as directed by their team. Development and completion of all required documentation as noted by MN Statute 245D, including, but not limited to, Service plan agreements, policy and procedures overview, individual abuse prevention plans, etc. Provide training to DSPs regarding each person’s service plans, outcomes, and other person-specific procedures and needs.
Qualifications:
- Must pass a DHS background check
- Possess a MN driver’s license (pass an MVA)
- Able to read and write English proficiently
- Familiar with MS Office.
- Previous Direct Service Provision required
- Previous supervision experience preferred but not required.
Direct Service Providers – 1 FT and 1 PT/Call-Up @ Tracy and 2-3 FT @ Tyler
Job Summary: Provide direct care to individuals with intellectual and/or developmental disabilities by attending to their needs as assessed by their individual support plans, addendums, and outcomes. No previous experience is required.
Qualifications:
- Be an example of courteousness, kindness and empathy to both staff and individuals
- Encourage community accessibility and inclusion by demonstration of manners, opportunities, suggestions, and promotion of each individual’s abilities
- Must pass a DHS background check
